Job Summary
A Care Coordinator at Steward Health Care is responsible for organising and overseeing the delivery of high-quality, person-centred care services for adults aged 18+ with diverse needs. The role focuses on ensuring care is delivered efficiently, safely, and in line with individual care plans while promoting independence, dignity, and overall well-being. Key Responsibilities
Care Coordination & Planning

Develop, implement, and review personalised care plans in collaboration with clients, families, and healthcare professionals

Schedule and coordinate care visits to ensure timely and effective service delivery

Ensure care packages are tailored to meet individual needs and preferences
Staff Support & Supervision
-
Allocate care staff to clients and manage rotas to ensure adequate coverage

Provide guidance, support, and supervision to care workers

Assist with onboarding and ongoing performance monitoring of care staff
Client & Stakeholder Communication

Act as the main point of contact for clients, families, and external professionals

Build positive relationships and respond promptly to queries or concerns

Liaise with healthcare providers and community services to ensure coordinated care
Quality Assurance & Monitoring

Monitor the quality of care being delivered and address any issues or risks

Maintain accurate and up-to-date records, reports, and documentation

Ensure care delivery aligns with company standards and regulatory requirements
Health, Safety & Compliance

Follow safeguarding policies and procedures at all times

Ensure confidentiality and promote dignity and respect for all clients

Adhere to health and safety regulations and company policies
Skills and Requirements
Essential

Desirable

Previous experience in a care setting (preferably in a coordination or supervisory role)

Relevant qualifications such as NVQ/QCF Level 3 in Health and Social Care or above

Experience with care planning or scheduling systems

Full UK driving licence (preferred for roles involving travel)
